Username is the same as your cpanel username. If you are trying to use an FTP account you created then use the username@yourdomain.com
Password is the same as your cpanel password. If you are trying to use an FTP account you created use the password you entered during the setup.
Hostname/Server name is ftp.yourdomain.com (replace yourdomain.com with your actual domain). If your account is new or the DNS has not been transferd then you may need to use your server's IP. This should be in your welcome letter or listed on the left hand side of your cPanel as "Shared IP Address"
